RN Case Manager Hospice (RN) Full-Time - $15,000 Sign-On Bonus

Sign-On Bonus $15,000

The Registered Nurse plans, organizes and directs homecare services/hospice services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

  • Completes initial, comprehensive and ongoing comprehensive assessment of patient and family to determine homecare/hospice needs. Provides a complete physical assessment and history of current and previous illness (es).

  • Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act

  • Assesses and evaluates patient's status by:

    • Writing and initiating plan of care

    • Regularly re-evaluating patient and family/caregiver needs

    • Participating in revising the plan of care as necessary

  • Initiate and Develop proper care plan for patients and include family/caregiver using health assessment data to determine/establish nursing diagnosis, making necessary revisions as patient status and needs change, and:

    • Incorporating palliative nursing actions

  • Administers medications and treatments as prescribed by the physician

    • And the Physicians Plan of Care

  • Promptly inform the physician or advanced practice nurse prescriber and other personnel participating in the patient's care in the patient's condition and needs.

  • Provides health care instructions to the patient as appropriate per assessment and plan of care.

  • Acts as Case Manager when assigned by the Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

  • Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.

  • Regularly assessing the patient's nursing needs, implementing the plan of care provision to meet those needs and re-evaluating the patient's nursing needs.

  • Acts as Case Manager when assigned by the Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

  • Participates in On-Call duties as defined by the On-Call Policy.

  • Hospice - Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group.

  • Assists the patient and family/caregiver and other team members in providing continuity of care.

  • Works in cooperation with the family/caregiver and hospice/homecare interdisciplinary group to meet the emotional needs of the patient and family/caregiver.

  • Supervises and teaching other ancillary personnel including licensed practical nurses and nurse aides. Delegates responsibilities when required. DHS 131.25 (4)(a)(2)


  1. Communicates with community health related persons to coordinate the care plan.

  2. Ensures that arrangements for equipment and other necessary items and services are available.

  3. Instructs, supervises and evaluates home health aide care provided every two (2) weeks.

Additional Duties
1. Fulfills the obligation of requested and/or accepted case assignments.
2. Actively participates in quality assessment performance improvement teams and activities.
3. Visit notes, time sheets, evaluations and other patient documentation submitted timely per agency policy.

Position Qualifications

  • Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting.

  • Current licensure in State of Wisconsin.

  • One (1) year of home health/hospice care experience preferred.

  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

  • Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.

  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
